Information on Lost and Stolen U.S. Passports
American citizens whose passports have been lost or stolen should first report this to the local police. Yemeni police generally require that the loss of the passport be advertised in a local Arabic-language newspaper before they will issue a police report. Applicants for replacement passports should bring this report, together with the Statement Regarding Lost or Stolen Passport, Form DS-64, a completed Application for Passport, Form DS-11, passport photos, and fees totaling $100 for applicants 16 or over and $85 for applicants under 16. Although the Embassy strongly encourages American citizens to report lost or stolen passports as soon as possible, once a passport is reported lost or stolen to the Embassy, it can no longer be used for travel, even if it is subsequently found or returned. Once approved, applicants can pick up their replacement passports from the Embassy in two weeks.
NOTE: BOTH parents must sign the passport applications of children under the age of 14.
