CHECKLIST FOR FIRST-TIME PASSPORT APPLICATIONS
If only one parent is an American citizen and you are applying for your child’s first passport, you must have… (Originals and photocopies)
- Passport Application DS-11
- Consular Report of Birth Abroad Application
- 2 passport photos
* 2 inches x 2 inches (or 5 cm x 5 cm) White background
* Picture and face size should match diagram on DS-11 application
* DO NOT staple photos to the application
- All Current and expired Old Passports of American Citizen Parent
- Naturalization Certificate or American Birth Certificate of American Citizen Parent
- Identification Papers of Non-American Parent
- Original Marriage Contract
- Original Birth Certificate of child
- Original Death/Divorce Certificates of Previous Spouses of Either Parent
- Passports and Original Consular Reports of Birth Abroad of Other Children (if applicable)
- Affidavit of Residency and Paternity/Maternity
* Fill out with EXACT DATES of periods in the U.S.
* Include all children and indicate their respective mothers/fathers
- Proof of Physical Presence in the U.S.
For children born after November 6, 1986, the American citizen parent must prove that he/she was physically present in the U.S. for 5 years. 2 of those years must be after the age of 14.
For children born before November 6, 1986, the American citizen parent must prove 10 years of physical presence in the U.S. 5 of those years must be after the age of 14
Proof includes, but is not limited to the following:
Tax Returns with W-2s can be taken as proof of physical presence if they are supported by secondary evidence. For instance, an employment with the exact date of employment can serve as secondary evidence.
School Transcripts and/or School Report Cards
Savings Account Bank Books
Credit Card Transactions - Bank statements
Leases Rent Receipts
Utility Bills in the Name of the American Citizen
Medical Records
Pay Stubs/ Slips
NOTE: Seamen MUST present discharge slips
- Proof of relationship between child and parents
Family Photographs
Family Identity Book
IMPORTANT: Both parents must be present to sign the passport applications of children under age 14. If one parent cannot be present a notarized letter of consent must be presented to the Consular Officer.

Execution Fee US$30
Consular Report of Birth Abroad US$65
Adult Passport Renewal US$55
Adult First Ten Year Passport Application US$55 plus US$30 (Execution Fee)
Child’s First Time Passport Issuance & Renewal US$40 plus $30 (Execution fee)
Please note that there will be $US 12 surcharge to all of the above passport fees.
Please mark the checklist as you acquire the requested documentation. Please do not come to the Embassy until you have everything that is required. We can only accept the applications of those who come with a completed checklist. Passport applicants should drop off their applications at the gate at 70:30a.m. and return with their family at 11:00a.m. for the interview.
All Arabic Documents MUST be accompanied by English translation.
NOTE: Passport issuance from Washington DC will take 2 – 3 weeks.
