Additional Visa Pages

Additional visa pages will not be added to an expired, damaged or mutilated passport, or to passports that were issued with 1 year or less validity. No more than two sets of additional visa pages will generally be added to any one passport.

Completing the application

Please type or print legibly in black ink when completing all sections of this application.

Please disregard instructions on the online forms, and submit the following to the US Embassy in Sanaa:

1. Fully completed and signed application form (Form DS-4085) (PDF_60 KB).
2. Current US passport;
3. Applicants under age 16: One parent is required to sign the application. Please submit the child's original birth certificate (plus translation) and the current identification of the parent signing the application, such as a valid passport;
4. Applicable fee;
5. Make an appointment to submit your application at the US Embassy in Sanaa.
