CHECKLIST FOR FIRST TIME U.S. PASSPORT APPLICATION
Laws governing transmission requirements: For children born before November 14, 1986, the US citizen parent must prove 10 years of physical presence in the US, including at least 5 years after the age of 14. For children born after November 14, 1986, the US citizen parent must prove 5 years of physical presence in the US, including at least 2 years after 14.
Both parents must be present to sign the passport application of children under age 14. If one parent cannot be present, a notarized letter of consent with a copy of the ID giving details of the second parent must be presented to the Consular Officer.
Proof of physical presence in USA: To prove physical presence in the US, the US citizen parent may use a combination of the following public documents: school records (sealed and signed), employment letters, W2s, tax returns (if self-employed, additional proof like Social Security statements or an IRS verification), pay stubs, medical records, savings account bank books, leases and rent receipts, and/or INS file.
Documentary requirements for all applications: All applications must include the proof of US physical presence listed above, the application forms listed below, and originals and copies of the US citizen parent’s current and old passports showing entries/exits (Yemeni and U.S.), naturalization certificate or U.S. birth certificate of American citizen parent, photo identification of non-American parent with translation, and all birth, marriage, divorce and death certificates (with English translation) of all individuals in the U.S. citizen parent’s family.
Required application forms: All applications must include a completed Consular Report of Birth Abroad application DS-2029 (first and second pages – including SS-5), passport application form DS-11, Affidavit of Residency and Paternity and 2 passport photos (2x2 inches with white background and face front, both ears and eye brows showing). Proof of relationship between child and parents: Applicants must prove the relationship between the child and the US citizen parent, which may be done with a combination of long form birth certificate with the parents’ names listed, old family photographs, family identity book, pre/post hospital records, and medical records of visits to doctors.
Please do not request an appointment from the Embassy until you have everything that is required. We can only accept applications of those who come with the complete documents.
